Completing the new patient registration ahead of time lets us spend more of your visit on you. Download the forms, fill them out and bring them with you, or call and we will help.
What should I bring to my first dental appointment?
Bring your completed forms, a photo ID, your insurance card if you have one, a list of medications and allergies, and any recent X-rays from a previous dentist.
How long is a new patient appointment?
Plan for about an hour. That covers your imaging, a thorough exam and time to talk through your goals and options with the dentist.
